My child has my permission to ride in the Bellville Methodist Church Van or Bus from school to Children’s Choir at Bellville Methodist Church, or other Choir related events with a church approved, and Safe Sanctuary trained Adult. I understand that the church approved, Safe Sanctuary trained adult will be alone with my child while in transportation to Bellville Methodist Church or other Choir related event or activity. I understand that by signing this waiver I release Bellville Methodist Church and any employees/volunteers of Bellville Methodist Church from any liability that may occur while my child is in route to Bellville Methodist Church or other Choir related event or activity with any of the church approved, Safe Sanctuary trained adults.
